About the numbers above

Cagrilintide is a long-acting amylin analogue. In trials it has been studied on its own and in a fixed combination with semaglutide (CagriSema). It is investigational and not approved, and vials sold online are sold for research use.

The trial ladder ran 0.3, 0.6, 1.2, 2.4 and 4.5 mg once weekly, stepping every four weeks. The table above uses that ladder. A 10 mg vial with 2 mL of water puts 0.3 mg at 6 units, which is a small draw; using 1 mL of water instead halves it to 3 units and is harder to read, so for the low rungs more water is the easier setup.

How long it stays in you

The half-life of Cagrilintide is about 7.5 days. That is the time for the amount in your blood to halve, so after one half-life you are at 50 percent of the peak, after two you are at 25 percent, and after four you are at roughly 6 percent. This is why a weekly compound still has meaningful levels present when the next dose lands, and why levels build over the first several weeks before they settle.

Common questions

How many units is 0.3 mg of Cagrilintide?
It depends entirely on how much water you added. In a 10 mg vial reconstituted with 2 mL, the concentration is 5 mg/mL, so 0.3 mg is 0.06 mL, which is 6 units on a U-100 insulin syringe. In the same vial with 1 mL of water it is 3 units, and with 3 mL it is 9 units.
How much bacteriostatic water for a 10 mg Cagrilintide vial?
2 mL is the usual choice. It gives 5 mg/mL, which puts the commonly reported range of 0.16 mg to 4.5 mg between roughly 3.2 and 90 units. Aim for a draw between about 10 and 50 units, where the barrel is easiest to read, and add or remove water to get there.
How many doses are in a 10 mg vial?
Divide 10 by your dose. At 0.3 mg that is 33 doses, which at once weekly is 231 days.
Can cagrilintide be dosed in the same syringe as semaglutide?
People report drawing both from separate vials into one syringe. Whether that is appropriate is a question for a clinician. The calculator only handles one vial at a time, so run each compound separately and add the two unit counts if you do combine them.
